We're here to answer your questions...
PHC Tech stands for Primary Health Care Technology. As a technology company, we recognise the function of the primary health care system as the unifying and integrating force in patient care. Therefore, our designs keep primary health care at their centre, and leverage how the primary health care system works to connect our care together.
We are concerned about both your privacy and your digital safety. We respect that your health information is extremely personal, and that you, like us, are apprehensive about information being stole, or being misused.
It is therefore important to us to secure your information, and, to protect you from your deidentified information becoming re-identified when compared with external databases. After all, we want to feel as safe as you do, when we upload our own data onto our platform to improve our health care and wellbeing.
To achieve this, we utilise multiple layers of security to protect you. We have partnered with industry cyber security experts to help us understand our risks and create a more secure digital environment. We use highly secure third-party providers to ensure our products are built on reputable foundations. And, for all patient users, we encrypt personally identifiable information on our platform’s backend configuration – meaning that any platform administrators, employees, or contractors cannot identify you. In the same way that passwords are encrypted and not seen by platform administrators, we cannot see your email, name, date of birth, address, phone number, names of your spouses or dependents, and cannot open or access your personal documents.
When you share your care record with other platform users (e.g. your clinician or family member), they will be able to see your personally identifiable information. However, when we share information with other health organisations to help them improve their health services and products (e.g. sharing data with health networks), we aggregate de-identified data, to ensure you cannot be reidentified.
For more information, please see our privacy policy which can be through the link below.

Our Care. Connected and My Health Record (the Australian Government's national digital health record platform) work together to help you navigate your health information.
Our Care. Connected and My Health Record are similar but distinctly different products. My Health Record functions to collect your information from multiple participating sources and functions as a repository for all your discharge summaries, health summaries, results, and etc. While this information is valuable, it can be difficult to reconcile and understand.
Our Care. Connected functions to help patients and clinicians see health information in an organised, up to date, and relevant format, which is easy to view and edit. Furthermore, the platform joins together your care with your care team by incorporating easily to use tools that improve clinician-to-clinician integration and cooperation.
We therefore can work synergistically with My Health Record data, to display and interpret your information simply, while allowing your care team to join together your care, all in the one location.
(Please note, at this stage, Our Care. Connected cannot import data directly from My Health Record)
Our platform is currently designed for individuals who can provide consent. Therefore, children under the age of 16 are currently not permitted to use our platform. However, a parent or legal guardian can create a profile for a child in their legal care if they desire to do so but will need to create a unique email account for each new user.
You can also create an account for your parent, provided that they consent to this profile being created, or, you are an authorised legal representative for your parent. You cannot create a profile for your parent or any other person without the legal authority or permission to do so.
We continue to work on the design of our platform and aim to expand how profiles are setup for families. Due to the delicate nature of how information sharing relationships can change over time, we are carefully consulting with industry experts to ensure children who use or have their data on the platform remain safe and protected.
Unfortunately, not. Your email address for your professional profile must be different from your personal profile. This is to ensure that your personal information cannot be exposed through your use of a professional user account.
Transient medical conditions are illnesses or diseases that are very common, short lived, and often not impactful on your complete health picture. They are also conditions that can make reviewing your health information cumbersome because they take up space and offer limited value. Our health records would be very long indeed if every occurrence of a cough or runny nose over our lifetime was listed!
To ensure our care records remain meaningful and easy to navigate, we offer two paths for you to categorise transient medical conditions. Firstly, you can have the event automatically archived so it is only viewable if you are looking for the entire history of your medical conditions. Secondly, you can re-categorise the condition as a recurrent condition and have it remain on your care record. For example, if you enter urinary tract infection or UTI, and you experience this condition frequently, you can easily re-categorise this condition as recurrent UTI, or recurrent urinary tract infection.
The current list of transient medical conditions is small and includes:
Bronchiolitis
Bronchitis
Conjunctivitis
Costochondritis
Fall
Folliculitis
Foodborne Intoxication (aka. food poisoning)
Gastroenteritis (aka. gastro)
Headache
Hepatitis A Infection
Nasopharyngitis
Otitis externa (aka. outer ear infection)
Otitis media (aka. middle ear infection)
Pharyngitis
Sinusitis
Upper Respiratory Tract Infection
Urinary Tract Infection
This list will change over time as we continue to conduct focus groups and track how users interact with our care record configuration.
Our primary function is to improve your health and wellbeing through strengthening the connection between the health system and between the health and community services sector.
Improving these connections is vital to delivering better health care, achieving better health outcomes, and responding more rapidly to changing health environments. Accordingly, we generate revenue by helping health organisations understand the health populations they serve, and, by improving their ability to communicate on our platform to strengthen care connections.
As mentioned above, our data sharing policy has strict provisions to protect your identity and ensure that your identity cannot be revealed after data has been shared.
We are busy preparing behind the scenes for our next update to better connect you to the digital health landscape, making it easier for you to connect and understand your results and related data.
We are also continuing to improve our privacy and user account designs to make our platform more accessible regardless of age, ability, or digital literacy.
